WAIVER AND RELEASE OF LIABILITY

I, the undersigned parent/legal guardian, acknowledge that participation in soccer training and related athletic activities involves inherent risks, including but not limited to falls, collisions, physical exertion, and other risks that may result in injury, illness, or property damage.

On behalf of myself and the participant, I voluntarily assume all risks associated with participation in any First Touch Soccer private training session. I hereby release, waive, discharge, and hold harmless First Touch Soccer, its owners, directors, employees, coaches, contractors, affiliates, facility partners, and representatives from any and all claims, demands, liabilities, damages, costs, or causes of action arising out of or related to participation in the program, including those resulting from ordinary negligence.

I certify that the participant is physically able to participate in soccer activities and that I am responsible for obtaining appropriate medical care if needed. In the event of a medical emergency, I authorize First Touch Soccer personnel to seek emergency medical treatment for the participant if I cannot be reached.

I grant permission for First Touch Soccer to use photographs and/or video footage of the participant for promotional, marketing, and educational purposes without compensation, unless I notify First Touch Soccer in writing.

I understand that private lesson fees are non-refundable unless otherwise approved by First Touch Soccer. Requests to reschedule must be made at least 24 hours prior to the scheduled session whenever possible.
